Last year, research published in Advances in Urology deemed “woman on top” (or as most of us call it, “cowgirl” or “reverse cowgirl”) the most dangerous position for men as it leaves the penis most susceptible to fracture. Sound awful? It is (trust me, I know. More on that later.) Researchers’ analysis found positions with women on top were the cause of half of hospital visits due to penile fracture. Coming in second was the deeply beloved doggystyle, accounting for 29 percent of visits, followed by “man on top” (or “missionary”) with 21 percent. Not queasy yet? Good. It gets worse.

A broken penis, scientifically known as a penile fracture, occurs when the tubes required to maintain an erection bend with a loud and painful pop after being thrust against a hard object, like a partner’s pubic bone. Researchers affirm men in their 20s or 30s are more likely to suffer the affliction, as they’re generally more fit and possess stronger erections.

But don’t let this scare you out of having sex. Keep in mind that penile fractures are extremely rare. Researchers could only find 44 penile fractures for this study, which speaks to how uncommon they are.

“Our hypothesis is that when woman is on top she usually controls the movement with her entire body weight landing on the erect penis, not being able to interrupt it when the penis suffers a wrong way penetration, because the harm is usually minor in woman with no pain but major in the penis,” researchers said. "On the contrary, when the man is controlling the movement, he has better chances of stopping the penetration energy in response to the pain related to the penis harm, minimising it.”

This year, a new position has dethroned cowgirl as most-deadly-to-your-penis. It’s last year’s runner-up: Doggy-style.

According to research published this summer in the International Journal of Impotence Research, which boasted a sample twice the size of the previous, more men have visited the ER with penile fractures due to doggy-style than any other position. However, the most dangerous positions articulated last year remained consistent. Doggy was followed by missionary, then, get this, “overzealous masturbation” and cowgirl, Lad Bible reports.

How does one avoid the horrors of a penile fracture? Thankfully, the New York Post recently spoke with sex expert Samantha Evans and asked her to speak to the potential dangers of each sex position as well as how to avoid them. Here are her tips concerning the worst possible positions for your penis.

Doggystyle: According to Evans, both parties need to slow the eff down and not get carried away by the carnal passion. Not only does this cause uncomfortable friction for the woman, but the male is also susceptible to injury if “the action is too fast and his manhood slips out and hits her pubic area.” Her advice? “Go slow and use a firm mattress.”

Missionary: Basically, overthrusting is a guy’s only issue here, which, again, would result in a penile fracture. For women, there’s a “danger of potential back pain for the woman if her back is not supported, and an uncomfortable entry angle can also be painful.” To remedy this, Evans advises you use pillow to raise her hips and support the back. I’ve personally found this maneuver encourages deeper penetration as well, so it’s a good idea either way.

Cowgirl: “Beware of bouncing too hard as it can be painful for the woman,” Evans explains, before warning men that “there’s a risk of penis fracture if the woman puts her weight on the man’s exposed member.” Which, is kind of what the whole position is about. What can you do to ensure you don’t break your dick? “Take time and stay in control to prevent slippage.”

Now is probably a good time to share that I once fractured my penis, as I alluded to earlier. It occurred just last year, on New Years Eve Eve. If you want to read all about my own experience with penile fracture as well as its subsequent horrors, I wrote about it here.
