us recent color snapshots or Polaroids that clearly show face and figure,
personal data such as age, height, weight and measurements, and copies of
two forms of ID (birth certificate, driver's license, passport or school
ID. One piece of ID must include a current photo). Submissions will not be
returned unless accompanied by a self-addressed, stamped envelope. E-mail
