Photos in any of the following formats: original color or black and white prints that are 5x7 or larger, original transparencies, original source TIFFs or JPEG files from a digital camera burned onto CD-ROM, DVD-ROM or copied onto a zip disk. These materials will not be returned to you.

Proof that you are 18 or older. Legible photocopies of two pieces of identification verifying your age, such as driver's license, passport or birth certificate. At least one must be a photo ID.
